All companies contracting sponsorship must complete and submit and original Application Contract. Application Contracts sent by fax, not properly completed, not signed or that fail to include a form of payment will be refused. Each company will be notified with a confirmation of sponsorship upon receipt by the MDA office. The MDA reserves the right to reject any sponsorship submissions based on conflict of MDA policies.

2015 Sponsorship Prospectus General Information

Display Space: Tabletop display space only will be available for sponsorship per fees as listed on a daily basis. Note no floor displays will
be allowed in display space. Assignments of tabletop space will be finalized by the MDA Annual Session Committee, Chairman: Dr. Joe Burns.
All correspondence and requests should be sent to the Mississippi Dental Association.

Registration of On-site Company Representatives: All company representatives who are on-site for the 2015 MDA Annual
Session are required to wear official badges supplied by the MDA. Advance registration badges will be issued only to employees of sponsoring
companies who are registered by the firm’s contact person. Registration is allowed for up to two (2) persons at no additional charge, with a
maximum of three (3) persons per company. Please note that an administrative fee of $25 will be charged for the registration of a third
representative. Sponsors are required to provide proof of employment by contracted companies when registering or requesting replacement
badges onsite. Only persons legitimately staffing sponsorships booths will be issued sponsor badges.

Cancellation: Cancellations received in writing through April 15, 2015 will receive a refund less a 50% administrative charge. Cancellations
received after April 15, 2015 will not be eligible for a refund.
